Benign prostatic hyperplasia (BPH) affects over 50% of men older than 60. Holmium laser enucleation of the prostate — HoLEP — has established itself as the gold standard for large prostates. Both EAU and AUA guidelines recommend it as a first-line option regardless of prostate size.
