Today I performed three live surgical cases for the InaSER Masterclass in Bali, Indonesia. The session was dedicated to teaching advanced endoscopic enucleation techniques. We utilized the Quanta System laser for all procedures. The live cases demonstrated the HoLEP method in a real clinical environment. Attendees from the region observed the surgical workflow directly. I explained specific technical steps during the intervention, focusing on apical dissection and mucosal preservation. The event served as a practical training module for local urologists. The cases were recorded and made available for review in my youtube channel.

I conducted a 2-day workshop on HoLEP at Ngoehra Public Hospital in Bali, Indonesia. The course included lectures on technique simplification and hands-on training for the participants. Participants practiced the dissection steps using the Holmium laser system. Local urologists from the region attended the event to learn new skills. We covered patient selection and procedural nuances in detail. The session focused on practical experience sharing among the attendees. The hospital facilities supported the training requirements effectively. Several complex cases were discussed during the theoretical part. The feedback from the participants was positive.

I spent the weekend at Hill Clinic teaching urologists from Turkey en bloc HoLEP. The group included new friends interested in the technique. We focused on the recent advances in Anatomic Enucleation using the Holmium laser. I explained how the procedure is simplified and how to perform early apical dissection. Live surgical cases were discussed to clarify the steps for mucosal preservation. The participants asked specific questions regarding the laser settings and tissue handling.

We completed the training session on Sunday afternoon.

I updated my recording setup this week. I used the same capture card but connected it to a Mac. I added a simultaneous audio track using Rode Wireless Pro microphones. I also implemented OBS Studio for Mac to manage the stream.

The whole setup is relatively simple and affordable. Grok helped me configure the software. This allows me to record surgical cases or lectures more efficiently. I tested the audio quality during a recent demonstration. The system runs smoothly on the Mac platform. I plan to use this for future webcasts and training sessions.

I performed two live surgical cases involving HoLEP today. Dr. Kumar moderated the session. He is one of the most experienced enucleators globally, having completed more than 15,000 HoLEP procedures. The case highlighted specific technical nuances in endoscopic enucleation. The live format allowed observers to view the procedure in real-time. Technical aspects of the laser system were explained throughout the enucleation. The interaction between the surgeon and the moderator facilitated the session. This session provided a detailed look at the surgical technique in a live environment. It was great to see Dr. Kumar who I met many years ago in India.

I recently reviewed the full text of a paper by Prof. Mario Sofer and colleagues on Anatomic Enucleation of the Prostate (AEEP). They propose a new mechanism to explain temporary stress incontinence after the procedure. The study suggests radial forces exerted by the scope during enucleation contribute to this outcome. This is an important read for all enucleators. It details the mechanical interaction involved in the technique. The work adds to the current understanding of patient outcomes following AEEP. I found the concept regarding radial forces particularly interesting for clinical practice.

Today I had Spanish visitors from different centers in the operating room. We had the collaboration of an Italian intern and the nurses from ICUAurologia. During the surgical session, I successfully performed six HoLEP procedures. I am grateful for the logistical and technological support provided by ItalFarmacoes for the equipment and laser. The procedure focused on prostate enucleation using holmium technology. It was an intense day with the participation of nursing staff and visiting professionals. The activity allowed us to share technical experiences in real time with the international team. Technical assistance ensured the efficient use of the laser during all procedures.

I am the only European surgeon not attending ICE25. I am participating in the Japanese event instead. My talk will cover specific surgical protocols. Today I performed another live case transmission to Medellín, Colombia. The procedure was part of a urology symposium at UROCLIN, celebrating 25 years of urological excellence at the institution. I collaborated directly with the team at HillClinic for the surgical procedure. The operation utilized the Quanta System laser for the case. The case was broadcast live to the urologists attending the symposium. I ensured the video feed was stable throughout the broadcast. This continues our series of live cases for international colleagues.

I performed a live En bloc HoLEP procedure today from the Hill Clinic in Sofia, Bulgaria. The surgery was transmitted live to the Philippines Urological Association Global Urologic Summit. The HoLEP technique was broadcast directly to the attendees. I demonstrated the en bloc technique during the session. The transmission allowed urologists from the Philippines to observe the procedure directly.

This was a standard surgical day shared with an international audience. The laser system was used for the enucleation of the prostate. This specific technique focuses on removing the adenoma in one piece. The procedure was completed successfully without complications. The live feed connected the operating room with the summit venue.

Next September I travel to Brazil to participate at the BEM meeting. I will be presenting on HoLEP techniques. The congress gathers urologists from the region to discuss surgical advances. I demonstrated the en bloc dissection using the Holmium laser. The visitors observed the live procedure and discussed the technical steps. We focused on practical application during the session. The group specifically reviewed the apical dissection phase. I appreciate the partner institutions for facilitating this training. It was a useful exchange of surgical knowledge. The visitors gained direct experience of a very polished surgical process and teamwork with the technique.
