My professional journey: 32 years in urology, 52 countries, 10.000+ procedures

I have spent 32 years in urology. I began my residency at Hospital de la Princesa in Madrid in 1994, completed my training at the Institute of Urology in London under Prof. Mundy, and in 2002 founded ICUA with my brother Santiago. We currently operate across 6 hospitals in Madrid, Cádiz, Jaén and Seville.

Since then, my career has revolved around two things: operating and teaching. I have performed over 10,000 laser prostate enucleation procedures — first with GreenLight (photoselective vaporisation and GreenLEP), then with holmium (en bloc HoLEP with early apical release). I have operated or provided training in 52 countries, and more than 600 urologists from around the world have visited our operating theatre to learn the technique.

The technical evolution

My path in laser prostate surgery has followed a constant evolution. I started with GreenLight photoselective vaporisation in 2003, developed the GreenLEP technique (green laser enucleation) which I first presented as a video at the EAU congress in 2010 and published as a full paper in World Journal of Urology in 2015, and since 2011 have been working with the holmium laser to perfect en bloc enucleation with early apical release — the technique we perform daily at ICUA and have published in series of 137, 754, and 6,193 patients.

Training as a mission

I have always believed that every urology department in the world should be able to offer endoscopic enucleation to its patients. Since 2007, our training programme at ICUA has received over 600 visiting urologists. Many of them now perform enucleation routinely at their own centres, and several have published their own results with the technique.

In 2010 I began my collaboration with the Hill Clinic in Sofia, Bulgaria, which has become another reference centre for enucleation training, with over 4,000 laser procedures performed there.

The manual

In 2026 I published the En Bloc HoLEP Manual (ISBN 978-84-09-81888-4), a 352-page book born from the questions and challenges that visiting surgeons raise in the operating theatre. It is not an evidence-based manual in the classical sense — it is an experience-based manual drawn from over 10,000 cases.
