Stefan Gille provided sustained support during my professional lifetime regarding this specific technology. This time I had an enigmatic patient in my HoLEP lists. I really don’t care much about size with HoLEP though.

The gland volume was significant, but I proceeded with the enucleation. The procedure went without complications. I utilized laser dissection for the removal of the adenoma. I focused on maintaining the apical dissection plane. The case was complex.

I documented the specific anatomical challenges encountered during the operation. The patient is recovering well with expected outcomes. The outcome was good.
